An extension of Unity Hospital, our short-term rehab center is a 40-bed unit focusing on treatment and rehabilitation following hospitalization. We offer the acute care our patients require for their recovery without being in a hospital setting.

As a Long-Term Care Registered Nurse, you will provide high quality and compassionate care to patients, residents and families in your unit while supporting continuity of care across all shifts.

STATUS: Full Time

LOCATION: Park Ridge Living Center

DEPARTMENT: Transitional Care Center

SCHEDULE: 3:00pm - 11:30pm, 11:00pm-7:30am

  • Patient Care & Service. Promote and restore patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; perform various treatment procedures; provide physical, educational and emotional support to patients, friends and families; supervise assigned team members

  • Planning & Communication. Develop and document individualized care plans customized for each patient’s unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress

  • Team Management. Assume responsibility and accountability on the unit and for the patient care team; work collaboratively with others

  • Compliance. Adhere to organizational policies, procedures and protocols, including policies governing safety, medication administration and documentation; complete required number of continuing education credits per year and mandatory in-services and health screens
